By completing this form, you give us permission to use your story across our media including website and social as well as external media outlets.
I hereby grant permission to use of Names, Business name and Logo in a success story, case study, interviews, testimonials or quotes, video, advertising collateral and any other material produced by The Growth Hub or agents acting on their behalf.
I grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video tape without payment or any other consideration.
Any of the materials mentioned in this approval may be displayed, reproduced, and referred to in presentations, on websites and in other marketing situations and at events, the Growth Hub will own all copyrights to the success story, case study, the video and any other material produced by The Growth Hub or agents acting on their behalf.
All text/material can be used in whole or in part without further permission in the media and formats outlined in this approval, including but not limited to social media, the press, reports.
I understand that my image and written material may be edited, copied, exhibited, published or distributed in all formats across platforms including though not restricted to presentations, online, print, press, advertising, social and social advertising shared with our funders and partners for their use. I waive the right to inspect or approve the finished product wherein my likeness appears.
Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording.
I also understand that this material may be used worldwide in perpetuity in all media known now or in the future. This release applies to photographic, audio or video recordings as well as written case studies, quotes or interviews collected by The Growth Hub or agents acting on their behalf. You may cancel this approval in whole or in part by giving a written cancellation notice to The Growth Hub. Upon receipt of such cancellation notice, we will endeavour to swiftly remove the published material from our website and from any future publications or communications.
By signing this form, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organisation utilising this material.
